Managing Medication Adverse Effects Post Kidney Transplant

Family History

Father died at age 39 polycystic kidney disease.

Mother died age 91 congestive heart failure.

Siblings:
2 sisters 1 died uterine cancer,
1 brother polycystic kidney disease, status post renal transplant.

Medication List

At the time of discharge from 2nd cadaveric renal transplant:

  1. Tacrolimus  4 mg po q12h generic
  2. CellCept® 1000 mg po q12h
  3. Prednisone 60 mg daily and then taper by 5 mg every 3 days, until on 10 mg every day
  4. Metoprolol 75 mg po 2 x day
  5. Procardia® 30 mg po 2 x day
  6. Allopurinol 100 mg po every day
  7. Prilosec® 20 mg po 2 x day x 3 months
  8. Bactrim® single strength one tablet po every day x 3 months
  9. Nystatin 5 cc swish and swallow 4 x day x 3 months
  10. Valcyte® 450 mg po every day x 6 months
  11. Natal care vitamins 1 mg po every day x 3 months
  12. Percocet® 5/325 one to two tablets po q 4-6 hr prn
  13. K-Phos® neutral 250 mg orally four times daily
