Progress Note 6 Month 8

Physical Therapy

Therapeutic exercise: stood independently for 76 seconds.

Transfers: supine to sit to stand with contact guard assistance using ½ kneel for transitions.

Gait: crawling up and down stairs independently; walking with supervision with rolling walker for 800 feet, steering difficulties; able to walk in parallel bars for 5 steps without holding on; coordination is improving but is ataxic.


Occupational Therapy

Gross motor: reaching and crossing midline with stand by assistance for trunk support; walking with hand-held assistance.

Fine motor: cutting with scissors with good bilateral skills; using tripod grasp for writing.


Speech Therapy

Reading: wrote story on lined paper; maximum cues for capitalization and punctuation; minimum cues for spelling; able to read one chapter of a book independently with maximum cues to go back for skipped lines, but no words were missed; she exhibited good comprehension of the story.

Verbalization: named six items in a category independently; assigned correct adjective to picture.

Problem solving: able to order & recall facts about each month; able to answer “wh” questions.

Math: able to add and subtract in word problems with 100% accuracy; two-digit math problems, requires maximum cues for borrowing, otherwise, minimum cues needed; beginning to tell time.

Memory: recall 5 out of 8 months accurately in order, minimum cueing for others; recalls the days of the week in order with cues.
