Posterior Fossa Syndrome

Progress Note 5 Month 7

Chemo treatments caused Katie to be ill for a considerable amount of time, resulting in numerous therapy cancellations.

Physical Therapy

Therapeutic exercise: able to squat, perform heel raises and balance activities independently; standing independently for 1 minute.

Transfers: floor to toilet SBA.

Gait: speed & balance improving during ambulation with rolling walker for 1500 feet, minimum assistance at shoulder girdle; able to walk 2-3 steps with supervision.


Occupational Therapy

Gross motor: max A cues to pull pants down during dressing; unable to unbutton pants; sitting & throwing bags at a target; bending at waist to pick up cones.

Fine motor: bead stringing for 10 minutes; tripod grasp; theraputty activities: picking items out of putty.


Speech Therapy

Reading: able to write her name legibly and long words with minimum to moderate cues; requires minimum assistance for skipped lines and word during reading activities; maximum assistance to summarize the story.

Verbalization: generate five items in a category with minimum cues.

Problem solving: describes similarities and differences with 90% accuracy.

Math: basic math still requires minimum assistance.

Memory: can identify coins by names and value with 75% accuracy; requires minimum cues for memory games.
