On examination, Mr. Fowler is apprehensive and is sweating profusely. Blood pressure is 140/100 mmHg and pulse is 70 per minute. Respiratory rate is 20 per minute. An EKG is performed (Exhibit 1). Brief physical examination reveals clear lung fields and no neck vein distention. On cardiac auscultation, he has an S4 with no murmurs.
