Mrs. Kell's surgical history includes an appendectomy at age 29, a hysterectomy at 47 and the removal of a benign tumor from her left lung at age 68.
She wears a hearing aid in the left ear and is deaf in the right ear. She wears eyeglasses and full dentures. The left eye has been "lazy" since an accident that occurred when she was 3 years old. Mrs. Kell had a cataract removed from the left eye 12 years ago.
Mrs. Kell was diagnosed with Parkinson's Disease at 73 years of age. She has taken thyroid medication for over 20 years. She has managed her diabetes, diagnosed over 10 years ago, with diet alone. She reports being allergic to tomatoes and tomato products, with no known drug allergies.
Three years ago Mrs. Kell experienced idiopathic left-sided hemiparesis for which she was hospitalized for 4 days. During her hospitalization she received occupational therapy, physical therapy and speech therapy services and a home evaluation was completed prior to discharge. No outpatient or home health rehabilitation services were required, as she demonstrated nearly complete recovery from the CVA prior to discharge.
