Mrs. Kell

Clinical Impression and Plan of Action


Mrs. Kell is likely in end stages of Parkinson's Disease with obvious depression. Mr. Kell is the primary caregiver and is expressing concern as to his ability to continue in that role. With Mr. and Mrs. Kell present the daughter requested involvement by a social worker and clergy member to assist the family with caregiver assistance and end of life issues.


The interdisciplinary team, including the patient's family, has been called together to develop a plan of action for this patient. Are there "other" problems that may be underlying or associated with her diagnosis? What might those "other" problems be? Print the IGA form. Acting as the team leader, record what you know on the IGA form and develop a plan of action for this patient. When you have completed your IGA form, compare it to the form completed by the CIGAP team.
