Interdisciplinary Geriatric Assessment:
Mrs. Curtis

Medical History


After Mrs. Curtis was admitted to the ER, the following medical history was provided by her daughter.

Allergies: None known to any drugs.

Habits: No use of tobacco, alcoholic beverages, and no history of IV drug abuse.

Hospitalizations: Approximately 3 months ago for back pain related to a thoracic compression fracture.

Mrs CurtisInjuries: 35 yrs ago MVA resulted in right wrist fracture, left lower leg fracture. Left hip fracture from a fall 10 yrs ago.

Surgery: Appendectomy when she was 21 y/o. Open reduction/internal fixation of hip fracture 10 yrs ago. Cataract extraction and placement of intraocular lens OS five years ago.

Illness: She has taken thyroid medicine off and on for over 25 yrs, and resumed this 9 months ago after not taking thyroid medicine for several years. A thoracic aortic aneurysm was detected during her prior hospitalization, and surgical repair was deemed inadvisable. She had gallstones with two or three bouts of acute cholelithiasis over the past four decades. Osteoporosis was diagnosed during her prior hospitalization.

Diet: She eats a general diet and has no dietary restrictions.

Medications & immunizations: Levothyroxine sodium 0.05 mg qd, Calcium 500 mg bid, and Aspirin bid PRN for an occasional backache. Has not received flu nor a pneumonia vaccination in the past year. No Td booster for more than ten years.
